Hello, and welcome! I am Ana, a dually certified Family and Psychiatric Mental Health Nurse Practitioner.
My journey in healthcare is rooted in my background. Born and raised in New York City as a first-generation Dominican American, I grew up understanding firsthand the unique triumphs and struggles that come with navigating different cultural worlds and facing life's complex challenges. I am fully bilingual and proud to offer care in both English and Spanish.
I completed both my Bachelor of Science and Master of Science in Nursing at Pace University, graduating summa cum laude. For over two decades, I have had the privilege of caring for diverse populations across the country, practicing in both New York and Los Angeles.
During my 20-year career as a Family Nurse Practitioner, I gained broad experience across several specialties, including family practice, dermatology, and community health settings serving underserved populations.
In each of these settings, regardless of the patient's primary physical concern, a profound truth became clear to me: emotional and mental well-being is the foundation of all health.
I saw firsthand that physical healing cannot be separated from peace of mind. Driven by this realization, I decided to return to school to deepen my expertise, earning my post-master's Psychiatric Nurse Practitioner certification from The Sage Colleges in Albany, New York.
Outside of my practice, I love exploring the world through travel and spending time with my family, including my husband, my daughter, and my mother. I am also passionate about health and wellness in my own life, and I enjoy creating new healthy recipes.
I look forward to meeting you and supporting your wellness journey.

If a medication is prescribed, it is sent electronically to your local pharmacy for easy pickup or delivery.

Scope of services and limitations
There are limits to what telehealth can safely treat.
Mindful Wellness NP PLLC provides medication management and telehealth services within specific safety parameters. For the wellbeing of our patients, we are not equipped to provide services to individuals experiencing:
- Suicidal intent or having a specific plan
- Psychotic symptoms or episodes
- Active self-harm behaviors
- Acute mental health crises requiring immediate intervention
Our commitment to your care
Your safety and appropriate level of care are our primary concerns. Therefore:
- If during intake, or individual session, we determine you need a higher level of support, we will assist in connecting you with appropriate in-person or virtual mental health services.
- You must immediately notify us if your mental health needs change, during the course of treatment, particularly if you begin experiencing any of the conditions listed above.
- We reserve the right to provide referrals to traditional in-person counseling services when we believe it is in your best interest.
Client acknowledgment
By engaging in services with Mindful Wellness, you acknowledge understanding of these limitations and agree to notify us of any changes in your mental health status that may require a higher level of care.
